If better leadership produces better results why don't more companies have a formal leadership development program in place?

The question was asked first on LinkedIn where other answers can be found. My answer was:

Perhaps the real problem is that the actual "leaders" aren't. Then the problem becomes learning how to be acceptable followers. Training in following might be the novel idea whose time has come.
